A common complaint of patients is a lack of energy. The lack of energy slows the healing time for back pain as well as decreases the initiative for performing home care such as stretching. An overlooked cause of the lack of energy is a condition known as non-alcoholic fatty liver disease, (NAFLD.) In 2023 the European Society of Cardiology reports that “This disease now effects almost 25% of the world’s population and is mainly observed in obese and type 2 diabetes.”
The liver performs more than 500 biochemical reactions and functions. This includes detoxification, immunity function, absorption of vitamins and minerals and energy production and storage.
According to the European Society of Cardiology, NAFLD involves elevated lipids, liver immune disturbances accompanied by insulin resistance within the liver, systemic insulin resistance (pre-diabetes), gut dysbiosis (altered gut microbiome) and systemic metabolic disorder (several functional problems with organs).
Routine laboratory tests that suggest this diagnosis are elevated AST and AST and with or without insulin resistance (pre-diabetes).
A diet in high fructose and a high caloric diet and I might add ultra- processed foods changes the gut microbiome which causes liver inflammation. Under this condition the liver cannot effectively perform its more than 500 biochemical reactions causing fatigue and general un-wellness.
Presently there are no effective drug therapies. The paper cites managing NAFLD with lifestyle changes including an increase in fruits and vegetables (not citrus fruits and other high fructose fruits). The most researched diet that would be appropriate is a Mediterranean Diet. The paper also cites the use of probiotics and exercise.
